Curriculum Request Form

This form can be used for inquiries regarding implementation discrepancies for programs or courses on the Course Catalog in myUFL (e.g., an approved prerequisite is not displaying correctly in the Schedule of Courses, the department that owns a course wishes to transfer or share ownership of that course, a course was approved for S/U Grading but is still displaying as Letter Grading, etc.).
