Curriculum Request Forms

Use this form (in myUFL) to alert registrar staff of discrepancies in programs or courses in the course catalog (e.g., an approved prerequisite is not displaying correctly in the Schedule of Courses, the department that owns a course wishes to transfer or share ownership of that course, a course was approved for S/U grading but is still displaying as letter grading, etc.).
